From the 14th of May 2024, Three Chequers Medical Practice will be piloting SystmConnect, a tool which allows patients to submit certain online requests to the Practice.
Update: 02/08/2024- We have succesfully completed the initial pilot stage where we offered Fit notes (Med3's) via this platform. As part of the next phase of roll out, we have added two further request types to the platform:
1. Update your contact details/address
2. Request Non-NHS work (Forms/Reports/Letters)

Do I have to use SystmConnect?
No, SystmConnect is completely optional and is intended as an alternative option for submitting certain requests to the Practice. You can still contact the surgery by telephone to make these requests if you are unable to submit an online form.
