Travel Information

We want to give our patients the best travel service we can.  In order to do this we need adequate advance notice of travel.  Please follow the advice below for a smooth service.

The 3 Important Timing Steps For A Successful Service - We Need 7 Weeks Warning In Total

  1. The Initial Travel Consultation must be booked for at least SIX WEEKS in advance of travel
  2. A risk assessment form must be submitted at least one week BEFORE the Initial Travel Consultation
  3. The first vaccine appointment must be at least ONE MONTH in advance of the date of travel

In order to avoid disappointment you must call us on 01722 336 441 or visit your surgery Reception to book in for your Initial Travel Consultation at least SIX WEEKS in advance of travel, WITH your completed Travel Risk Assessment Form (or you can fill one in there and then).   If you ring us to book your appointment, please ensure you hand in a completed Travel Risk Assessment Form at least one week before your Initial Travel Consultation.  

Please note that handing in a submitted travel risk assessment form alone does NOT mean you have booked an Initial Travel Consultation.  

Please hand in your completed travel risk assessment form a FULL WEEK before your Initial Travel Consultation.  Some vaccines take a month to give as a full course and we often need to order specialist vaccines, which can take a working week, so your Initial Travel Consultation must allow SIX WEEKS before the date of travel.

Please download the Risk Assessment Form, print and complete it, then take it to your surgery Reception and hand it in when you book your Initial Travel Consultation.  You can also come in and fill in a form in the surgery if you wish.

Please be aware that some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS. 

We may have to order vaccines we do not have in stock and this can take up valuable time.  If forms are received later than six weeks in advance we regret to say we may not be able to cover you for your trip.
